Acenocoumarol

  • Adverse Effect an increased risk of bleeding
  • Clinical Management In patients receiving Acenocoumarol , the prothrombin time (PT) or international normalized ratio (INR) should be closely monitored with the addition and withdrawal of treatment with miconazole, and should be reassessed periodically during concurrent therapy. Adjustments of the acenocoumarol dose may be necessary .

    Amphotericin B

  • Adverse Effect Pharmacodynamic antagonism between these agents
  • Clinical Management Monitor patients for therapeutic effectiveness if these medications are given concomitantly.

    Glimepiride

  • Adverse Effect severe hypoglycemia
  • Clinical Management During concurrent therapy, monitor blood glucose closely and observe for signs and symptoms of hypoglycemia (eg; fatigue, restlessness, malaise, irritability, weakness, increased perspiration).

    Phenindione

  • Adverse Effect An increased risk of bleeding
  • Clinical Management In patients receiving Phenindione , the prothrombin time (PT) or international normalized ratio (INR) should be closely monitored with the addition and withdrawal of treatment with miconazole, and should be reassessed periodically during concurrent therapy. Adjustments of the phenindione dose may be necessary in order to maintain the desired level of anticoagulation.

    Phenytoin

  • Adverse Effect an increased risk of phenytoin toxicity (ataxia, hyperreflexia, nystagmus, tremor)
  • Clinical Management Avoid systemic imidazole antifungal therapy if possible. Monitor patient closely for signs or symptoms of phenytoin toxicity. Serum level monitoring of phenytoin may be helpful interpreting suspected toxicity.

    Warfarin

  • Adverse Effect an increased risk of bleeding
  • Clinical Management: In patients receiving Miconazole with warfarin, the prothrombin time (PT) or international normalized ratio (INR) should be closely monitored with the addition and withdrawal of treatment with miconazole, and should be reassessed periodically during concurrent therapy.
