Buspirone
Adverse Effect: An elevated SGPT (ALT) level Clinical Management: During concurrent therapy with trazodone and buspirone, monitor liver function closely.
Carbamazepine
Adverse Effect: An increased risk of carbamazepine toxicity (ataxia, nystagmus, diplopia, headache, vomiting, apnea, seizures) Clinical Management: Monitor for symptoms of carbamazepine toxicity when trazodone is added to therapy. Serum carbamazepine levels should also be considered with the addition or discontinuation of Trazadone and dosage adjustments made accordingly.
Fluoxetine
Adverse Effect: trazodone toxicity (sedation, dry mouth, urinary retention) or serotonin syndrome (hypertension, hyperthermia, myoclonus, mental status changes) Clinical Management: Due to the potential for impairment in trazodone metabolism, patients should be monitored for any signs of trazodone toxicity. Occasional dosage reductions of trazodone may be required.
Fluphenazine
Adverse Effect: Hypotension Clinical Management: Monitor blood pressure, particularly in patients who might be sensitive to this effect. Advise patient to rise slowly from lying or sitting position.
Haloperidol
Adverse Effect: An increase in plasma concentrations of m-chlorophenylpiperazine (m-CPP), the active metabolite of trazodone
Thioridazine
Adverse Effect: Hypotension Clinical Management: Monitor blood pressure, particularly in patients who might be sensitive to this effect. Advise patient to rise slowly from lying or sitting position.
Tramadol
Adverse Effect: CNS depression (sedation, lethargy, speech difficulties) Clinical Management: Use caution if tramadol is to be used concomitantly in patients receiving trazodone therapy. Monitor patients for signs of CNS depression (sedation, lethargy, speech difficulties) and adjust doses accordingly. The manufacturer recommends that reduced dosages of tramadol be used when it is coadministered with other CNS depressants.
Trifluoperazine
Adverse Effect: Hypotension Clinical Management: Monitor blood pressure, particularly in patients who might be sensitive to this effect. Advise patient to rise slowly from lying or sitting position |
