Oropharyngeal, esophageal candidiasis Neonate > 14 day, infant, child: 6 mg/kg PO or IV on day 1, then 3 mg/kg for at least 2 wk.
Adult: 200 mg PO or IV on day 1, then 100 mg PO or IV once qd. Up to 400 mg qd for esophageal disease. Cont treatment for at least 2 wk after symptoms resolve.
Systemic candidiasis Neonate > 14 day, infant, child: 6-12 mg/kg/day PO or IV.
Adult: Up to 400 mg PO or IV once qd. Cont treatment for at least 2 wk after symptoms resolve.
Cryptococcal meningitis Infant, child: 12 mg/kg PO or IV on day 1, then 6 mg/kg once qd × 10-12 wk after CSF culture negative. To suppress relapse in pts with AIDS, 6 mg/kg once qd.
Adult: 400 mg IV or PO on day 1, then 200 mg once qd. Cont treatment 10-12 wk after CSF culture negative. To suppress relapse in pts with AIDS, 200 mg once qd.
Prophylaxis against mucocutaneous candidiasis, cryptococcosis, coccidioidomycosis, histoplasmosis in pts with HIV Infant, child: 3-6 mg/kg PO qd.
Adult: 200-400 mg PO or IV qd. |
