Dosages:
  • Nebulisation: 3-5ml of a 20% solution 3-4 times daily. If required 1-10 ml of a 20% solution can be given in every 2-6 hours. Mechanical suction of liquefied secretions may be necessary.    
  • Instillation: 1-2ml of 20% solution can also be given by direct endotracheal instillation as often as every hour.    
  • Oral: 200 mg thrice daily.    
  • IV Infusion: Paracetamol overdosage: Initially 150mg/kg body weight of Acetylcysteine in 200ml of 5% glucose is given over a period of 15 minutes followed by 50mg/kg in 500ml of glucose over the next 4 hours and then 100mg/kg in 1 100ml over the next 16 hrs. Treatment is very effective if the drug is administered within 8 hours following Paracetamol poisoning.    
  • Management of unstable angina pectoris: 15 minutes after Nitroglycerin infusion Acetyl cysteine in a dose of 5g in 200mlof 5% dextrose has to be infused by slow I V perfusion for a period of 15 minutes. Supplement infusion of 2ml of Acetyl cysteine along with 2ml of normal saline and 2-3% of acetic acid has to be repeated every 6 hours for 24 hours.
